SUPPORT SERVICES

We have a team of Care Co-ordinators who can offer their advice and services for older people. Our commitment to you, will offer you advice and access to relevant information to enable you and support you to solve difficulties or provide you with the details of advocacy support services that may be able to help.

By having this service available to you helps and supports you to exercise your rights and makes sure your voice is heard. We can point you in the right direction if you require any help with the following: Welfare benefits Housing and Property Matters. Respite care, Social Care and Health organisations and Social Activities and Personal matters.

We appreciate that people feel safer and more comfortable in their own home. We can arrange care and support following a hospital stay. We will conduct an assessment and design a person-centered care plan ready for your discharge, so that you feel supported when you return to your home. We will take into account all of your health needs, emotional well-being, preferences, wishes, and choices.

All our staff are fully training in administering medication and can assist you to take your medication from a chemist filled dossett box. If you have specialised needs you will be allocated a care worker trained in the specialised technique required and your care/support plan will be agreed with others involved in your health care, such as your district nurse who will assess your care worker and sign them as competent.

Supermarket shopping service, for people who have problems accessing a supermarket. Our Community Support Workers can either go to collect your shopping for you, or they can accompany you to the shops.

This service also gives you an opportunity to socialise over a cup of tea and a snack in the supermarkets cafe.
